Survey of lived experience of mental health

The Victorian Royal Commission into Mental Health is seeking submissions from Victorians with a lived experience of mental health issues, their family and carers (formal and informal).

We want to put your lived experience front and centre in the feedback we plan to provide to the Royal Commission. By including your voice, Mind Australia and Wellways Australia want to help ensure that your experiences and ideas have an impact on the direction of mental health care in Victoria, and how the mental health sector can support better mental health outcomes.

Mind and Wellways are administering this survey, and the results will be shared with Launch Housing with your consent. You can review your consent at the end of the survey.

If you would rather put your responses in an email or Word document, or have any concerns about this survey, please send them to royalcommission@yourvoice.org.au. Please be aware that this would identify you.

You can also make comments directly to the Royal Commission website.

Privacy

We do not ask for your name or contact details in this survey, but do request your age and postcode at the end of the survey. There are significant privacy and confidentiality issues in disclosing health information, and we take seriously our responsibility to protect it. Information will be periodically removed from our online server and held more securely in local storage and destroyed at a later date. View a our Privacy Policy.

Any information submitted via email will have identifying information removed before being added to the dataset, after which our records of your email will be destroyed.

Responses required by Friday, 7 June 2019

For your responses to be considered as part of Mind and Wellways submissions to the Royal Commission, you need to have completed the survey by Close of Business Friday, 7 June 2019. However, our survey will remain open as there will be further opportunities to provide feedback to the Royal Commission throughout the second half of 2019.
